Welcome to the Ministry Health Care Student Affiliations web site.

The information provided here is designed to guide students who are seeking a learning experience within Ministry Health Care. Students must attend an institution of higher education and be enrolled in a program requiring applied learning objectives to be able to participate in a clinical or educational placement.

An Affiliation Agreement (Contract) must be in place between the school and Ministry Health Care. Please contact the Student Affiliations Coordinator at 715-343-3373 if you have questions regarding an existing contract or need to initiate a new contract.

The institution of higher education must provide an instructor/coordinator to sponsor the student(s) and to act as a liaison to Ministry Health Care.

Placement Steps

1. Requirements Form - Print, complete and return along with required health work at least 6 weeks prior to the desired start date of your placement.

2. Complete the on-line application form. Make sure you have your school, emergency contact, and placement information handy for you to refer to as this will be required on the application. PLEASE NOTE: A Ministry Student ID number will be assigned to you after you complete the on line application. You need to retain this number for completion of the Orientation materials and for any future placements you may need to add.

   * Nursing students will need the Ministry Student ID number to complete the computer documentation training which follows below.
   

3. Complete the Computer Based Training (CBT) General Orientation

For Nursing Students ONLY

All clinical rotations at Saint Clare’s Hospital and Saint Joseph’s Hospital require completion of the “computer documentation training” CBT’s prior to the first day of the clinical experience.

Please allow ample time to complete this requirement. Each of the 14 modules may take up to 15 minutes. Complete the training in its entirety. This will result in an auto generated notice back to Ministry Health Care verifying your completion status.

Returning Students
Complete the Placement Form only. This form will ask you for your Ministry Student ID Number. Be sure you have it with you before you begin to fill in your placement request.

Current Ministry Health Care Employees

If you are a current Ministry Health Care Employee, please complete the on-line application above and indicate that you are an employee in the "Comments" field. Have your Employee Health department contact Laura Bart at 715-343-3373 regarding your health work.
